I, for and in consideration of this emotional reset service or nutritional consultation which is hereby acknowledged, do hereby release and forever discharge Becki Siconolfi, Live Healthy With Becki, Vivre Sano, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to this coaching service.
I understand that after payment is submitted it is not eligible for refund.
I furthermore understand and agree that any no-call/no-show will result in the forfeit of all monies paid. I also understand that tardiness of more than 5 minutes for a session without prior communication will result in forfeiture of the session, and all monies paid. As the service provider, Becki Siconolfi requires the entire length of the session time booked to render her services, and if you show up later than 5 minutes late Becki Siconolfi is unable to perform the full benefit of the service, and therefore you understand you waive your right to a refund.
I understand Becki Siconolfi requires 24 hours notice for cancellations, or the full fee will be assessed.
Furthermore, I understand if I attempt to chargeback the session after services have been rendered, I will be banned from using any services ever again (coaching, subscriptions, supplements, programs). If the credit card company charges back, I understand I will still be responsible for payment which can be pursued in small claims court. I agree not to defame Becki Siconolfi, Live Healthy with Becki, and all extensions of Live Healthy with Becki (Vivre Sano Supplements) either directly or indirectly in writing, orally, or on any social media platform and you will be responsible for any civil damages as result of the defamation.
I understand it is my responsibility to disclose any medical or mental health issues upfront. I also understand I am booking an appointment for educational purposes only, and it is not talk therapy, counseling, or psychotherapy.
Prices may be subject to change at any time.
It is understood and agreed that this release shall be binding upon me, and my respective heirs, executors, administrators, personal representatives, successors, and assigns. This release shall be subject to and governed by the laws of the State of Pennsylvania.
